Early nutrition management in patients receiving extracorporeal membrane oxygenation (ECMO) remains controversial. Despite its potentially beneficial effect, enteral nutrition (EN) could be associated with gastrointestinal (GI) complications. Total daily energy requirements remain difficult to achieve with ECMO support. Analysis of nutrition practices could improve nutrition management of this particular population.
A monocentric retrospective study of patients requiring ECMO in a cardiac surgery intensive care unit (ICU) between 2010 and 2014 with follow-up ≥6 days. Nutrition support was monitored daily until ECMO weaning. We compared patients exposed (EN group, n = 49) and unexposed (No EN group (NEN), n = 63) with EN, as well as the energy and protein intakes within 4 days after initiation of ECMO. Vital status and nosocomial infections were followed up until ICU discharge. Primary outcome was the incidence of GI intolerance and risk-factor identification. Secondary outcomes included impact of nutrition inadequacy and clinical outcome.
A total 112 patients were analyzed, representing 969 nutrition days. Median ratio of energy and protein prescribed/required daily was 81% (58-113) and 56% (36-86), respectively. GI intolerance was experienced by 53% (26 of 49) of patients in the EN group and was only associated with ECMO duration (odds ratio, 1.14 95% CI, 1.00-1.31; P = .05). Low-energy and protein days were not associated with clinical outcomes such as nosocomial infections.

Because the occurrence records for this bird are old and inaccurate, we investigated the current and future environmental suitability of the 14 plant species used by C. spixii as resource. These plants are key elements for the long-term reestablishment of the species in the wild, so the use of models helps in the assessment of the effects of climate change on the availability of these resources for the species and informs selection of the best places for reintroduction. We based our models of environmental suitability on 19 bioclimatic variables and nine physical soil and topography variables. Climate projections were created for the present and for the year 2070 with an optimistic (SSP2-4.5) and a pessimistic (SSP5-8.5) climate scenario. Both future climate scenarios lead to a reduction in area of environmental suitability that overlapped for all the plant species 33% reduction for SSP2-4.5 and 63% reduction for SSP5-8.5. If our projections materialize, climate change could thus affect the distribution of key resources, and the maintenance of C. spixii would depend on restoration of degraded areas, especially riparian forests, and the preservation of already existing natural areas. The Caatinga domain is very threatened by habitat loss and, for the success of this reintroduction project, the parties involved must act to protect the species and the resources it uses.Policy Points States can create policies that provide access to publicly funded prenatal care for undocumented immigrants that garner support from diverse political coalitions. Even though nearly 6% of citizen babies born in the United States have at least one undocumented parent, undocumented immigrants are ineligible for most public health insurance. Prenatal care is a recommended health service that improves birth outcomes, and some states, including both traditionally “blue” and “red” states, have opted to provide publicly funded coverage for prenatal services for people who are otherwise ineligible due to immigration status. This article explores how courts and legislatures in three states have approached the question of publicly funded prenatal care for undocumented immigrants and its relationship to thverge on health policies affecting access to care for undocumented immigrants. Future research should explore the reception of various message frames for expanding public health insurance coverage to immigrants in other contexts.

Sensory perceptions are controlled by specialized neuronal circuits established through fine-tuning, which relies largely on neuronal activity during the development. Spontaneous neuronal activity is an essential driving force of neuronal circuit refinement. At early developmental stages, sensory cortices display spontaneous activities originating from the periphery and characterized by correlated firing arranged spatially according to the modality. The firing patterns are reorganized over time and become sparse, which is typical for the mature brain. This review focuses mainly on rodent sensory cortices. First, the features of the spontaneous activities during early postnatal stages are described. Then, the developmental changes in the spatial organization of the spontaneous activities and the transition mechanisms involved are discussed. The identification of the principles controlling the spatial organization of spontaneous activities in the developing sensory cortex is essential to understand the self-organization process of neuronal circuits.
